Type 2 Diabetes Prevention and Wellbeing Event at Hanson Lane Enterprise Centre

To mark Type 2 Diabetes Week there will be a dedicated event held at Hanson Lane Enterprise Centre to raise awareness of the increased risks of the condition in the South Asian Community and offer guidance on building a healthier lifestyle.

Organised by Calderdale Cares Partnership and the NHS West Yorkshire Integrated Care Board, the event will shine a spotlight on the risk factors of type 2 diabetes, as well as the non-clinical measures individuals can take in reducing these risks and preventing the onset of type 2 diabetes.

At the event a number of partners will be present, including:

Reed Wellbeing

  • Height, Weight, and BMI measurements
  • ‘Know your risk tool’
  • Sign posting
  • Resources

Central Halifax PCN

  • Blood Pressure checks
  • Sign posting to Reed Wellbeing
  • Access to all 4 practices systmOne, so therefore could task referrals to NDPP if individual has a HbA1c reading between 42-47mmol within last 12 months
  • Signpost individuals back to practice for updated HbA1c and referral to NDPP

Diabetes UK

  • Type 2 risk resources
  • South Asian food resources
  • Online learning zone
  • All diabetes info ‘know your risk’

Outreach

  • Provide advice on south Asian portion sizes (plate portions, examples)
  • Healthy eating habits
  • Sample of foods
  • Recipes cards
  • Resources and billboard learning

GP – Robyn

  • Short presentation 10:30 – 11:30

Halifax Opportunities Trust – Tandrusti project team

  • Introduce the Tandrusti project
  • Raise awareness of risks of Type 2 diabetes in South Asian community
  • Launch free groups and activities available for local activities to support a healthier lifestyle
